Patient navigator (Patient Advocate):

The patient navigator is a health professional that focuses on navigating the patient through their COVID-19 isolation.   The navigator accomplishes this by collecting and documenting information from the patient and providing the necessary information for successful isolation.

Responsible for the following:

  • Receiving reports of persons confirmed as having COVID-19.
  • Interviewing identified people to determine if criteria has been met to be released from isolation.
  • Provide release of isolation documents via email or mail and update system database
  • Clearly and concisely gather information about symptoms, exposures and identifying others with whom might possibly be infectious.
  • Monitor and document the person’s compliance with isolation protocols
  • Enter information in a statewide database with the assurance of accuracy and detail.
  • Monitor incoming voicemail messages and direct to appropriate investigator.
  • Assist with updating database with investigator name and case status
  • Review reports to determine timely contact is made with each individual

Minimum Requirements:

  • Ability to manage, compile, analyze and present data.
  • Ability to communicate clearly, both orally and in writing with diverse audiences.
  • Ability to work with a variety of computer software packages.
  • Bachelor’s degree

Highly Desirable Qualifications:

  • Knowledge of public health practices, communicable diseases, disease control and epidemiologic methods.
  • Master’s of Public Health or related field
  • Understanding of medical terminology and principles of exposure, infection, infectious period, and symptoms of disease
  • Basic skills of crisis counseling and case management.
