Job Description

In a support capacity to a government agency, receives case referrals and performs intake and placement for eligible program participants:

  • Answers emergency hot-line calls and responds to an e-mail based resource inbox;
  • Receives and analyzes complex requests regarding vulnerable populations in order to place participants into the most appropriate level of care;
  • Determines type of action based on the demographic information and circumstances of each case as well as the availability of space at grantee programs;
  • Makes the initial determination/assessment on placement and informs the appropriate agency;
  • Documents and refers to appropriate person(s);
  • Maintains confidential files and other related records of cases;
  • Prepares shift report on any significant issues, transitions to and briefs next shift staffer on outstanding cases;
  • Coordinates and provides status updates on placements for the life cycle on the case intake process;
  • Responsible for input and verification of data for each case;
  • Reviews source documents for accuracy of input data using a system of quality assurance;
  • May be responsible for integrity of certain database information;
  • Investigates questionable data and takes corrective action when necessary;
  • Performs other administrative duties as needed

Education

  • Bachelor’s Degree in Social Work, Sociology, Psychology or a related discipline preferred.

Qualifications

  • With BA/BS degree, 2 years of relevant experience; with AA/AS degree, 4 years of experience
  • Excellent written and verbal communication skills
  • Experience working with immigrant, refugee, and underserved populations is preferred    
  • Strong attention to detail
  • Bilingual Spanish/English a plus
  • Strong organizational skills and expert user of MS Excel
  • Excels in a fast pace, high volume environment

Unique/Additional Requirements

  • Position includes rotating shift work 24/7 to include some telework
  • Requires Public Trust or the ability to obtain one
